Pleural effusions: Evaluation and management REVIEW
نویسنده
چکیده
Treatment depends on the severity and the cause. Thoracentesis is done to relieve symptoms. Chest tubes provide continuous drainage in cases of pneumothorax, hemothorax, penetrating chest trauma, complicated parapneumonic effusion or empyema, or chylothorax. Pleural sclerosis (pleurodesis) is usually indicated for patients with uncontrolled symptomatic malignant effusions. ANY CONDITIONS can cause pleural effusions, including diseases that are local (in the lungs or pleura), extrapulmonic, or systemic. In many cases the cause is a chronic condition for which the patient is already receiving treatment; therefore, a patient with pleural effusion may present to a pulmonologist—or to a general internist, other medical specialist, or surgeon. In up to 20% of cases the cause remains unknown despite a diagnostic workup.

The management of benign non-infective pleural effusions.
The evidence base concerning the management of benign pleural effusions has lagged behind that of malignant pleural effusions in which recent randomised trials are now informing current clinical practice and international guidelines. The causes of benign pleural effusions are broad, heterogenous and patients may benefit from individualised management targeted at both treating the underlying dise...
متن کاملEvaluation and outcomes of pediatric pleural effusions in over 10 years in Northwest, Iran
Abstract Background: Pleural effusion is the accumulation of excess fluid in the pleural cavity. Most information available about pleural effusion is obtained from studies on adults and little evidence is available in children. Therefore, it is necessary to identify the existing status and explain the disease process, signs, treatment, and prognosis. Children with pleural effusion who were admi...
متن کاملTh1/Th2 Imbalance and Elevated PD-L1 in Pleural Effusion Predict the Risk of Multi-Drug Resistant Tuberculous Pleuritis
Background: Patient immune status might be indicative of the variance in bacterial genetics in drug-resistant tuberculous pleuritis and could be used for predicting the risk of multi-drug resistant tuberculous pleuritis (MDR-TB). Objective: To determine the significance of Th2/Th1 ratio and concentration of PD-L1 in the pleural effusions for prediction of MDR-T...

Background & Aim: Adenosine deminase(ADA) can be used in the diagnosis of tuberculous pleural effusions. In countries with high prevalence of tuberculous pleural effusions, specificity and sensitivity for ADA test is high, therefore it is an integral part of a diagnostic workup of lymphocyte-rich exudative body fluids, and it is a cheap and economically cost-effective test. No study has bee...
متن کاملTh1 and Th2 Cytokine Profiles in Malignant Pleural Effusion
Background: The alteration of Th1 and Th2 cytokine levels is the subject of controversy in pleural effusions caused by malignancy, a situation that favors a Th2 immune response. Objective: To examine the different levels of IL-4 and IL-10 (Th2 cytokines), and IL-2 and interferon-γ (IFN-γ) (Th1 cytokines) in malignant and non-malignant pleural effusions. Method: The cytokine levels in pleural fl...
